209 - ABUSE REPORTING

DPI staff and security officers will uphold all state and federal rules regarding witnessing and reporting various levels of abuse. Abuse is defined as an action that causes harm or injury to another person. This can refer to:

  • Physical Abuse

  • Financial

  • Neglect

  • Involuntary seclusion

  • Sexual Abuse

  • Wrong Restraint

  • Verbal Abuse

Reporting

DPI staff and security officers will report suspected abuse to the appropriate authorities. Should protective services be needed, or a crime has been committed DPI will inform law enforcement in the county where the staff is making the report. They will work closely with all state agencies to collect all information necessary to make a complete and accurate report.

Employees will document alleged abuse in a report containing the following information:

  • Name, age and present location of the individual

  • The nature and extent of the alleged abuse, including any evidence of previous abuse of the

    individual or evidence of previous abuse by the person alleged to have committed the abuse

  • Information that led to report being filed

  • Other pertinent information that will be helpful in establishing the cause of alleged abuse and

    the identity of the person alleged to have committed the abuse

  • The date of the incident

Each report will be held in confidence. Investigation of abuse in relation to staff will be performed by an outside unbiased party to avoid any conflict of interest.
